Care Path Connections specializes in helping patients find the right care options. We tailor care options to individual care needs, preferences, financial ability and demographic area. We are dedicated to offering a range of pre-screened, quality care options in adult family homes, assisted living facilities, memory care facilities, and in-home care at no cost to our clients.
Our hands on case managing approach supports families from the beginning to the end of finding and securing the right care for their loved ones. We are committed to addressing each roadblock along the path to care with years of experience in navigating families through them. Additionally, and upon request, we provide real estate services to help clients leverage their assets for funding their care.
Our Approach
How We Work
Our approach involves prescreening and connecting with quality and diverse care providers, this is an ongoing process. We are always excited to meet new and quality care providers who are interested in becoming a contracted partner and quality option for our clients.
Next, we connect with families directly or through their care teams to complete a detailed intake process in order to understand what their circumstances are, and what they are wanting and needing in day care options. We approach our cases whole-heartedly, allowing our clients to determine the level of support that fits their comfort level. This means we can meet with clients in person, over the phone, or even virtually over zoom or by email.
Once we have completed our intake process, we follow through with our clients guiding them to visit and consider care options. We will remain in close contact, available to address any barriers to care, from liquidating assets to applying for our Grant. Care Path Advisors will accompany our clients every step of the way utilizing our years of experience and knowledge to navigate them down the path to securing long term care.
Once our clients have been placed into the care of their choice we will remain in contact. Following up with our clients is an important part of our regular process. We keep in touch even after successful placement to help our clients address any ongoing barriers to care. It is our ultimate goal to get placement right for our client's long-term care needs and to avoid repeating the need to cycle back into the care system.
Care Path Connections is excited to be building towards being able to offer the Care Path Connections Grant! We are currently committed to gathering donations to support patients who require financial assistance for their care. These donations will help us reach the goal of supporting selected applicants to pay for and secure long-term care with the Care Path Connections Grant. Our grant is designed to serve those who fall in between the cracks of qualifying for long term care coverage and being able to pay for their care privately.
